stripe double refund webhook and dashboard

The actual problem

Support refunds in Dashboard while automation also refunds, creating double payouts out.

Do this

1. Idempotency key refunds on order id; block second refund if already succeeded.
2. Train support to use one path (Dashboard XOR app) per order.
3. Alert when refund amount exceeds original charge.
4. Reconcile refund rows in the weekly payouts CSV.
